2. Book Online

Book your appointment online. Choose an available time and complete your intake forms before your visit.

You will complete:

  • Basic contact information

  • Reason for your visit

  • Medication and allergy information

  • Preferred pharmacy

  • Telehealth consent forms

  • Payment information

3. Join Your Virtual Appointment

You will receive instructions for your secure video visit.

At the start of the appointment, the provider will confirm:

  • Your identity

  • Your current location in Florida

  • Your emergency contact

  • Your privacy and consent for telehealth care

4. Complete Your Medical Evaluation

Your provider will review your concern, relevant history, medications, symptoms, and available records.

Because telehealth has limits, some conditions require an in-person examination, urgent care evaluation, emergency department assessment, diagnostic testing, or specialist referral.

5. Receive Your Care Plan

After the evaluation, you will receive clear next steps. Your plan may include:

  • Treatment recommendations

  • Prescription medication when clinically appropriate

  • Medication refill evaluation

  • Laboratory orders

  • Review of laboratory results

  • Chronic disease follow-up

  • Home monitoring instructions

  • Follow-up appointment

  • Referral for in-person or specialty care

Prescriptions, antibiotics, laboratory tests, or referrals are not guaranteed. The provider makes each decision after completing the clinical evaluation.

6. Follow Up When Needed

Non-members may schedule follow-up visits as needed.

Telehealth members receive:

  • Up to two scheduled telehealth visits per month

  • Secure messaging

  • Continued follow-up for appropriate health concerns

Additional member visits are available for $89 each.

Telehealth may be appropriate for:
