Telehealth Informed Consent

Purpose

This consent explains the telehealth services offered by Nexon Health Care LLC, including benefits, limitations, risks, alternatives, privacy protections, prescribing rules, and patient responsibilities.

Before telehealth care begins, Nexon Health Care LLC will obtain and document your consent through the intake process or at the start of the visit. Your acknowledgment forms part of your medical record.

Provider Disclosure

Your provider is Ernesto M. Sorribes, APRN, FNP-BC. He is a Florida-licensed Advanced Practice Registered Nurse, a board-certified Family Nurse Practitioner, and registered with the Florida Board of Nursing for autonomous practice in primary care.

He provides adult primary care within his education, training, certification, licensure, and lawful scope. He practices as an APRN and is not licensed as a physician in Florida. Autonomous practice permits an eligible Florida APRN to provide authorized primary care services without a physician supervisory protocol for services within the approved autonomous scope.

Eligibility and Location

Telehealth services are limited to adults age 18 or older who are physically located in Florida during each visit.

At the start of every telehealth visit, Nexon Health Care LLC will confirm your identity, exact physical address in Florida, callback number, emergency contact when clinically appropriate, and everyone present. The visit will not proceed if your identity, location, safety, or eligibility is not verified.

What Telehealth Involves

Telehealth uses live audio and video technology to deliver healthcare when the patient and provider are in different locations. Secure messaging, photographs, documents, questionnaires, and remote health information might support the visit.

A telehealth visit includes, as appropriate:

  • Review of symptoms, medical history, medications, allergies, and available records

  • Visual observation and patient-assisted examination

  • Review of home measurements, laboratory results, imaging, and outside records

  • Diagnosis and treatment planning

  • Medication review and prescribing when clinically appropriate and legally permitted

  • Orders for laboratory testing or imaging

  • Referrals and follow-up planning

  • Preventive and chronic disease care

The provider will diagnose, treat, or prescribe only after completing an evaluation sufficient for the presenting condition. If telehealth is insufficient, the provider will defer diagnosis or treatment and recommend an in-person visit, testing, urgent care, emergency care, or referral.

Nexon Health Care LLC offers telehealth visits by appointment.

Expected Benefits

  • Reduced exposure to infectious illness

Limitations and Risks

Telehealth lacks the full physical examination and diagnostic resources available during an in-person encounter. Possible risks include incomplete assessment, delayed diagnosis or treatment, need for in-person or emergency evaluation, poor image or sound quality, connection failure, interruption, miscommunication, unauthorized access despite safeguards, and reliance on information or measurements you provide.

Home measurements are affected by device accuracy and technique. Tell the provider if you are uncertain about a reading. The provider might request a repeat measurement, an office measurement, or other in-person testing.

Healthcare outcomes depend on multiple clinical and individual factors. A telehealth visit does not guarantee a diagnosis, prescription, laboratory order, referral, medical letter, work note, or specific result.

Technology Failure

If the audio or video connection fails, the provider will attempt to reconnect using the contact information in your record. Depending on the clinical situation, the provider will resume the secure visit, use limited telephone communication when appropriate, reschedule, arrange an in-person appointment, direct you to urgent or emergency care, or contact emergency services.

A telephone call does not replace video or in-person evaluation when either is required for safe care.

Privacy and Confidentiality

Nexon Health Care LLC uses administrative, technical, and physical safeguards designed to protect health information. No electronic system eliminates every privacy or cybersecurity risk.

Use a private location and a trusted device and network. Avoid public Wi-Fi when possible. Prevent others from seeing or hearing the visit unless you want them to participate. Your information remains subject to the Nexon Health Care LLC Notice of Privacy Practices and applicable federal and Florida law.

Do not send clinical details through ordinary email, a public Wix form, or social media. Use Elation Patient Passport or another secure method provided by the practice.

No Recording

Nexon Health Care LLC does not routinely record telehealth visits. No participant is permitted to audio-record, video-record, photograph, livestream, screenshot, or reproduce any part of a visit unless every participant gives prior express consent.

If recording is clinically necessary, Nexon Health Care LLC obtains separate express authorization describing the purpose, access, use, and retention of the recording.

Emergencies

Telehealth through Nexon Health Care LLC is not an emergency service and is not monitored continuously.

Call 911 or go to the nearest emergency department for chest pain, severe shortness of breath, stroke symptoms, severe bleeding, loss of consciousness, seizure, severe allergic reaction, suspected overdose, immediate danger, or another condition requiring immediate treatment.

Call or text 988 for a mental health, suicide, or substance-use crisis when no immediate physical danger exists. If an emergency develops during the visit, the provider might contact 911, emergency services, or your emergency contact using your confirmed location.

Prescriptions and Controlled Substances

A telehealth visit does not guarantee a prescription. Prescribing decisions depend on clinical judgment, available medical information, examination limits, medication history, applicable law, and patient safety.

Nexon Health Care LLC does not prescribe Schedule II controlled substances through telehealth. Other controlled substances, if any, are considered only when the prescriber holds all required state and federal authority, the evaluation is sufficient, and all Florida and federal requirements are satisfied.

The provider might require an in-person examination, laboratory testing, medical records, prescription-monitoring review, specialist evaluation, or another safety measure before prescribing.

Your Responsibilities

  • Provide complete and accurate health information

  • Report medications, allergies, pregnancy status, and relevant history

  • Confirm your exact Florida location and callback number at every visit

  • Use a private and safe setting

  • Do not drive or operate machinery during the visit

  • Identify every person present

  • Follow emergency and follow-up instructions

  • Complete recommended testing and referrals

  • Protect your portal credentials and device

  • Do not record the visit without every participant's prior express consent

Alternatives

You remain free to decline telehealth. Alternatives include an in-person visit with another qualified professional, urgent care, an emergency department, or delaying care with the understanding that delay might worsen the condition.

Right to Withdraw Consent

You have the right to withdraw consent for future telehealth services at any time through the Elation patient portal, in writing, or verbally before a visit begins. Withdrawal does not affect care already provided, records already created, lawful billing, record-retention, or reporting duties. Future care might need to occur in person or through another provider.

Separate Consent for Specific Care

This telehealth consent does not replace informed consent for a specific medication, procedure, test, treatment, disclosure, or other clinical intervention. The provider will discuss material risks, benefits, alternatives, and expected outcomes and obtain separate consent when required.

Electronic Acknowledgment

By acknowledging and signing this consent electronically through the intake process, you confirm that you are age 18 or older, will be physically located in Florida during every telehealth visit, received the provider disclosure, understand that the provider is an APRN and not a physician, understand the benefits, limitations, risks, alternatives, privacy terms, emergency limits, and prescribing rules, agree to identity and location verification, agree not to record without every participant's prior express consent, had an opportunity to ask questions, voluntarily consent to telehealth, and understand how to withdraw consent for future services.

Your acknowledgment record includes your name, signature or affirmative acknowledgment, date, time, and the consent version in effect.
